Forty-three states, the District of Columbia and five Canadian provinces currently offer some type of credential to professionals in the massage and bodywork field—usually licensure, certification or registration.[79][118] Thirty-eight states and the District of Columbia require some type of licensing for massage therapists.[119] In the US, 39 states use the National Certification Board for Therapeutic Massage and Bodywork's certification program as a basis for granting licenses either by rule or statute.[120] The National Board grants the designation Nationally Certified in Therapeutic Massage and Bodywork (NCTMB). There are two tests available and one can become certified through a portfolio process with equivalent training and experience.[121] Between 10% and 20% of towns or counties regulate the profession.[97] The National Certification offered by the NCBTMB does not mean that someone can practice massage in any state.[122] These local regulations can range from prohibition on opposite sex massage, fingerprinting and venereal checks from a doctor, to prohibition on house calls because of concern regarding sale of sexual services.[97][123]
That is, regardless of all other considerations, a massage therapist must talk to you about pressure, respect your preferences (they are more important than any treatment ideology), and be careful about stumbling into areas that need much less pressure (for comfort) or much more pressure (for satisfaction). Far too many therapists make the mistake of setting a “default” pressure for a client early on, and then using roughly that much pressure everywhere.

Pauline Wills, author of the Reflexology and Colour Therapy Workbook, teaches that colors can be applied to "areas where an abnormality has been diagnosed but which has produced no noticeable symptoms in the physical body." She states that the application can be done by imagining colors transmitted through the practitioner's hand or by Firstly, if the practitioner is sensitive to colour, they can visualize it being projected or by using "reflexology crystal torch." Many proponents claim that foot reflexology can cleanse the body of toxins, increase circulation, assist in weight loss, and improve the health of organs throughout the body. Others have reported success in treating earaches, anemia, bedwetting, bronchitis, convulsions in an infant, hemorrhoids, hiccups, deafness, hair loss, emphysema, prostate trouble, heart disease, overactive thyroid gland, kidney stones, liver trouble, rectal prolapse, undescended testicles, intestinal paralysis, cataracts, and hydrocephalus (a condition in which an excess of fluid surrounding the brain can cause pressure that damages the brain). Some claim to "balance energy and enhance healing elsewhere in the body." [2] One practitioner has even claimed to have lengthened a leg that was an inch shorter than the other. There is no scientific support for these assertions.

In Mexico massage therapists, called "sobadores", combine massage using oil or lotion[102] with a form of acupuncture and faith.[103] Sobadores are used to relieve digestive system problems as well as knee and back pain.[102][103] Many of these therapists work out of the back of a truck, with just a curtain for privacy.[103] By learning additional holistic healer's skills in addition to massage, the practitioner may become a curandero.[104]

Referred pain spreads the goodness. Undoubtedly another reason that massage pain can feel good is the phenomenon of referred sensation. If you stimulate internal tissues anywhere in the body, muscle or otherwise, the brain really has trouble telling quite where the sensation is coming from. When you press hard enough on your muscles, particularly on sensitive trigger points, the pain is often experienced as though it originated from a much broader area.

There’s rarely any justification for extremely painful massage, unless it clearly produces a better result than gentler treatment — which is rarely clear!1 It is possible that a few “brutal” deep tissue massages could do the trick where gentler treatment would fail — but there is no way to know this in advance, and massage is expensive stuff. If you’re going to gamble on a treatment, gamble on cheaper and less painful ones.


No one really knows how a painful massage can also feel so good at the same time. This is a sensory phenomenon mostly beyond the reach of science — not entirely14 — all we can do is speculate. A main question is whether good pain is good because we expect relief to follow pain, or because positive and negative qualities are being produced simultaneously. My bet is on the latter.

In another study, 35 women with premenstrual syndrome (PMS) were randomly assigned to ear, hand, and foot reflexology or to placebo therapy done on sham reflex points. The women kept a daily record of 38 possible symptoms selected from previous PMS research questionnaires. The treatment group reported significantly fewer symptoms than the placebo group, and these improvement persisted for 2 months after treatment. Many women in this group fell asleep during the 30-minute sessions and reported feeling more energetic during the next day. The placebo group reported that they thought they were receiving genuine reflexology, The authors note, however, that it was very difficult to develop a credible placebo control group, which may have been the study's flaw. Normally, reflexology is soothing, but the placebo treatment was described as "either overly light or very rough." [13] Thus the differences could have been differences in the quality of the massage being administered. The study suggests that massage may relieve PMS symptoms, but it does not validate the alleged connection between reflex points and body organs
